Include phone numbers of other persons involved, if applicable.
Include email addresses of other persons involved, if applicable.
For eg: Private swimming lessons, personal fitness training, private parkour lessons, semi private parkour lessons, jungle survival and junglecraft short course, universal wilderness survival and bushcraft long course, private self defense classes, cycling lessons, permaculture training, onsite parkour consultation service. Simply look at our website and see what’s the name(s)/description(s) of the service(s) you’re going for.
Please state all accurately, to the best of your knowledge, and do not falsely declare. If you’re not the person to be trained, do state on behalf of the other persons (to the best of your knowledge) and we’ll clarify with them again (if any of those persons are not your biological/legal child under the age of 18). Also state (to your best knowledge) on behalf of anyone else involved (we’ll double check with those persons again later). Separate (accordingly) all persons’ info with commas and full stops, where necessary.
